Medication Reconciliation Reflection

     I thought it was interesting to learn about my family members medication.  It was interesting to learn why the doctors prescribed them and what effects they had on the body.  Some safety concerns that I had for my patient came mostly from the two blood pressure medications (losartan and amlodipine) .  The main side effects of these drugs were dizziness, fatigue, and headaches.  However, some of the biggest concerns were related to alcohol usage and avoiding grapefruit juice.  These could cause a decrease in blood pressure and worsen side effects which was concerning.  These issues were not age related to my patient and they understood the protocols that had to be followed.  I also mentioned the importance of monitoring blood pressure periodically to make sure they were receiving the desired effects of the drugs.  Lastly, the patient understood why they were on these medications (blood pressure regulation) and knew that by not taking these medications that it increased their risk for stroke.  The patient was very good about this and took their medication at the same time every morning.  There were not any serious medication interactions between the four of these drugs (acetaminophen, calcium carbonate, amlodipine, and losartan) and the patient was aware of this.  The patient understood the risks of these drugs and well aware of the side effects to monitor for.
